By booking an appointment, attending a consultation, or undergoing treatment, you acknowledge that you have read, understood, and agreed to the following terms.
1. Scope of Services
Mumtaj Dental Care provides preventive, diagnostic, restorative, cosmetic, surgical, orthodontic, and other dental services in accordance with accepted professional standards and ethical dental practices.
2. Appointments and Scheduling
- Appointments are scheduled to ensure efficient patient care.
- Patients are requested to arrive on time for all appointments.
- The clinic reserves the right to reschedule appointments due to unforeseen circumstances, emergencies, or operational requirements.
- Repeated missed appointments or late cancellations may affect future appointment availability.
3. Medical History and Patient Responsibility
- Patients are responsible for providing complete, accurate, and updated medical and dental information.
- Any changes in medical conditions, medications, allergies, or pregnancy status must be disclosed promptly.
- The clinic shall not be held responsible for complications arising from inaccurate, incomplete, or undisclosed medical information.
4. Treatment Planning and Informed Consent
- Treatment recommendations are based on clinical examination, diagnostic records, radiographs, scans, and professional judgment.
- All proposed treatments, associated risks, benefits, alternatives, limitations, and estimated costs will be explained to the patient.
- Written informed consent may be required before specific procedures.
- Dentistry is a biological science, and individual treatment outcomes may vary. Therefore, no guarantee or warranty can be provided regarding the success or longevity of any treatment.
5. Financial Policy
- Fees for consultations, investigations, laboratory procedures, and treatments are payable as per the clinic’s prevailing fee schedule.
- Patients are responsible for settling all charges promptly.
- Treatment costs may vary if additional procedures become necessary during the course of treatment.
- Fees paid for completed professional services are generally non-refundable unless otherwise determined by clinic management.
6. Clinical Records and Documentation
- The clinic maintains comprehensive patient records, including treatment notes, radiographs, OPG images, intraoral scans, clinical photographs, and other diagnostic records.
- These records remain the property of the clinic and are maintained in accordance with applicable legal and professional requirements.
- Copies of records may be provided upon written request, subject to applicable regulations and administrative procedures.
7. Use of Clinical Images
- Clinical photographs, radiographs, and digital records may be used for treatment planning, documentation, academic presentations, professional education, and awareness activities only with the patient’s prior consent.
- Patient identity will be protected whenever possible.
8. Follow-Up Care and Patient Compliance
- Successful treatment outcomes depend on patient cooperation and adherence to prescribed instructions.
- Patients are expected to attend scheduled review and maintenance appointments.
- The clinic shall not be responsible for adverse outcomes resulting from failure to follow professional advice or recommended follow-up care.
9. Infection Control and Safety
- The clinic adheres to strict sterilization, infection-control, and hygiene protocols in accordance with accepted dental standards to ensure patient safety.
10. Emergency Care
- While every effort will be made to accommodate dental emergencies, immediate treatment cannot be guaranteed and remains subject to clinical priority and availability.
11. Limitation of Liability
- All dental services are rendered with reasonable skill, care, and professional competence.
- The clinic and its dental professionals shall not be liable for complications, treatment failures, or outcomes arising from biological variability, patient non-compliance, undisclosed medical conditions, or factors beyond reasonable professional control.
12. Privacy and Confidentiality
- Patient information shall be treated as confidential and handled in accordance with the clinic’s Privacy
- Policy and applicable legal requirements.
- Information will only be disclosed with patient authorization or where required by law.
13. Amendments
- Mumtaj Dental Care reserves the right to modify these Terms and Conditions at any time without prior notice.
- The latest version shall be available at the clinic and on the clinic’s official platforms.
14. Laws and Jurisdiction
- These Terms and Conditions shall be governed by and interpreted in accordance with the laws of India. Any dispute, claim, or legal proceeding arising out of or in connection with the services provided by the clinic, these Terms and Conditions, or any treatment rendered shall be subject to the exclusive jurisdiction of the competent courts at Cuddalore, Tamil Nadu.
Nothing contained herein shall restrict or limit any rights available to patients under applicable laws of India.
